Informant Worksheet
In the Commonwealth of Massachusetts, funeral directors are responsible for completing and filing the death certificate. In collaboration with other entities such as medical offices, the board of health, and the city/town clerk, funeral directors gather and collect the information needed for the biographical component of the death certificate.

Release Authorization
When a death occurs, some facilities and institutions require what is called a Release Authorization. This is to be signed by the legal next of kin or legal representative who gives permission to the listed facility to release their loved one into our care. Facilities like the hospitals and the medical examiner require these forms to be completed and sent to them before this can take place.

Cremation Authorization
In the Commonwealth of Massachusetts, funeral homes are not allowed to own or operate crematories. Funeral homes are to work with third party crematory providers to provide the services of cremation in supporting the wishes of those who have passed or the wishes of their loved one. We work with Walnut Grove Crematory in Danvers in providing the cremation process to our families we work with.

Identification Form
Our identification form is used as a record ensuring the proper identification of loved ones in our care. Whether family wishes to see their loved one, use identifying characteristics, or would like to use an identification bracelet from the facility a loved one passed away in, this form is a used in our process.
